Write an application for a duplicate insert. The office will give you a sample or a prepared form for such a statement. It is written in the name of the rector of the university, contains your data, number and series of the lost diploma. The statement must also indicate the reason for the loss of the diploma or insert.

In state educational institutions and universities that have passed accreditation, payment of a duplicate is required. Remember that the fee should not exceed double the cost of the form.

Take the application addressed to the rector, a police certificate and a receipt for payment to the office or the secretariat.

Some educational institutions require those who have lost a diploma or an insert to advertise in a newspaper. To do this, contact the local free edition and advertise as follows: "Diploma - indicate the number, series and year of graduation - consider invalid."

After a while - from a week to several months - you will be given a new insert. It will be labeled "duplicate." If, together with the insert, you have lost the diploma itself, the duplicate will have a new number and series. And the date of issue of the diploma will be the date of issue of the duplicate. The old document is canceled.

If you have lost the insert of the diploma issued before July 22, 1996, then you will not be given a duplicate insert. According to the law, in this case it is necessary to issue an extract from the working curriculum according to which you studied. It will indicate the studied disciplines and the period of study.
